Methods of Payment

Currently, I am in network with: 

• Anthem/Blue Cross, EAP

This company usually pays for your entire fee, i.e. there is no copay.

If you have an insurance plan that will reimburse you if you see an out-of-network provider, I will give you  "super bills" that you can submit to them for reimbursement.

DIRECT PAY

Your fee is determined based upon your ability to pay. Call or e-mail to set a fee.
Payable by cash, check, credit card,  PayPal or Venmo.
